Facial Feminization?

Facial Feminization Surgery (FFS) is often a combination of procedures that help create a more feminine appearance by changing the shape of the face. This surgical series can be performed on anyone who feels like their face isn’t feminine, however, it’s primarily used for those with gender dysphoria and who would like to transition physically to their self-affirmed gender. Some of the surgeries can be combined but this treatment is primarily performed on a step-by-step basis.

Facial feminization surgery often includes a combination of the following procedures:

  • Forehead Contouring: Most cisgender men possess a more pronounced bony ridge above their eye sockets, whereas cisgender women tend to have a higher, smoother, and more rounded outline of their forehead. Dr. Jaiswal will cut part of the forehead bone out, reshape it, and then put it back to achieve a more feminine appearance on the forehead.
  • Eye and Lid Modification: Dr. Jaiswal will cut away excess tissue to feminize the upper eyelids.
  • Cheek Augmentation: It’s common for cisgender women to have prominent round cheeks, due to a greater concentration of fat in the area. To achieve a more feminine appearance, fat can be taken from another part of the body (i.e. belly or thighs) and placed into the cheeks to alter their size and shape. In a bit more extreme technique, Dr. Jaiswal will fracture and move the cheekbones into a different position.
  • Lip Lift and Augmentation: A feminine face tends to have a shorter distance between the base of the nose and the border of the upper lip, while masculine faces usually have longer upper lips. By shortening the distance between the lip and the nose, Dr. Jaiswal can create a shorter, more curled lip. If desired, your lips can also be enlarged by taking fat from another part of the body or using implants or fillers and placing it in the lips.
  • Jaw Angle Reduction: Since masculine jaws are typically larger and broader than feminine jaws, Dr. Jaiswal will reduce the size of your jaw and narrow it by sculpting or removing outer layers from the lower jaw.
  • Chin Width Reduction: The difference between masculine and feminine chins is that the latter tend to be smaller and more square-shaped. To shorten and narrow the chin, Dr. Jaiswal will make a horizontal cut along the chin bone, followed by the removal of a small wedge of bone, and then reattaching the chin bone.

Other possible head and neck procedures include:

  • Tracheal Shave: This procedure will minimize the thyroid cartilage (a.k.a. Adam’s apple). During the procedure, a small incision is made just under the chin, in the shadow of the neck, or hidden in a skin fold to conceal the scar. Dr. Jaiswal will then reduce and reshape the cartilage.
  • Lowering the Hairline: Feminine hairlines tend to be lower, so an incision is made at the edge of the hairline and the scalp is raised, pulled back, and fixed to the bone. Any excess forehead skin will be removed.
  • Hair Transplantation: Hair transplantation can also be used to create a more feminine hairline. In this procedure, hair follicles are taken from the back and side of the head and then transplanted to bare areas of the head and the temples.

You might also consider skin-tightening surgery, such as a facelift or neck lift.

Who are the Best Candidates for Facial Feminization?

What to Expect on the Day of Facial Feminization

On the day of your Facial Feminization surgery, you will be made comfortable with either general anesthesia or intravenous sedation, depending on what is chosen during your consultation. Once you are sedated, Dr. Jaiswal will begin your surgery.

Every patient will require a combination and techniques to achieve their look, and Dr. Jaiswal strives to create a beautiful and feminine face. If you would like to understand what happens during each procedure, please differ to the information provided above in the “What is” section.

Facial Feminization Recovery

After surgery, we will monitor you in our recovery room as you come out of sedation. Once you are free to leave, a family member or friend can take you home. You can expect to experience some pain, redness, and swelling in your face, most noticeably in your eyes, nose, jaw, and chin. You will need to avoid overexerting yourself for at least one month, and once Dr. Jaiswal gives you the ‘OK’, you can begin moderate exercise within three to six months.

You will notice that chewing and swallowing may be difficult due to incisions and swelling, so you should stick to softer foods as you initially heal. If you’d like, we can offer our recommendations for nutritionists in the area to make sure you get enough nutrients following surgery, because a healthy diet is an important part of your recovery.

Facial Feminization FAQ’s

Is Facial Feminization Painful?

The surgery isn’t painful since it is performed under general anesthesia, but you can expect some pain and discomfort during your recovery. Dr. Jaiswal will provide you with instructions on how to alleviate any discomfort, as well as temporarily prescribe you pain medication.

How many appointments will I need?

The number of appointments you will need will be decided upon during your consultation. As mentioned above, while most procedures can be combined, Dr. Jaiswal may recommend that you break them up into several surgeries to achieve the best results.

After your surgeries, Dr. Jaiswal will want to see you for a follow-up about a week post-op, so he can monitor your healing and remove or replace any bandages and sutures. Depending on the procedures you get, you may require more additional check-ups than another patient. You will always be informed of when he expects to see you again and when your last appointment will be.

How long until I see results?

Results will vary for every patient, but depending on the degree of change and the procedures you get, you may see some positive changes immediately after surgery, however, you will need to wait for any swelling and bruising to subside. Many patients will see their final results within three to six months, but may also take up to a full year.

Can I combine Facial Feminization with other procedures?

Please keep in mind that due to the extensive nature of these procedures, you may be advised to receive additional surgery at a later date.
